COMPOSITION :

     Each film-coated tablets contains-

      Tadalafil IP.                             20mg
      Excipients.                                   q.s

DESCRIPTION:

      When a man is sexually aroused, the arteries in the penis expand and harden thus allowing more blood supply to the penis, causing an erection. Erectile Dysfunction, also called importance, is the inability for a man to achieve and maintain an erection sufficient for sexual activity. According to a report, this problem affects 31% of all adult males and 50% of all males over 40 years of age at some point in their life.
         Tadalafil 20mg or 10mg tablets are used to treat erectile dysfunction in man. It helps the penis fill with enough blood to cause an erection. After sex is over erection goes away.

PHARMACOLOGICAL ACTION:

           A.MECHANISM OF ACTION:

         Tadalafil is a phosphodiesterase enzyme(PDE5) inhibitor. This enzyme is responsible for the degradation of cyclic guanosine (cGMP) produced in reaction to sexual stimulation.
           The more cGMP is available. The tadalafil inhibits the PDE5 enzyme, preserving cGMP levels, therefore aiding the sexual activity and durability. Filling occurs because the blood vessels that bring blood to the penis/vagina and deliver more blood to the penis/vagina and at the same time, the blood vessels that take away from the penis/vagina remove less blood from the penis/vagina. Sexual stimulation that leads to a penile/vaginal flush causes the production and release of nitric oxide in the penis/vagina. The nitric oxide causes an enzyme, guanylate cyclase to produce cyclic guanosine monophosphate(cGMP). It is the cGMP that is primarily responsible for increasing and decrease the size of the blood vessels carrying blood to and from the penis/vagina respectively and causing the flush. When the cGMP is destroyed by another enzyme phosphodiesterase-5, the blood vessels return to their normal size, blood leaves the penile/vaginal area and the flush ends.
   

    B.PHARMACOKINETIC:

     Absorption- 

         Tadalafil is rapidly absorbed after oral administration and the mean maximum observed plasma concentration is achieved at a median time at 2 gours dosing. Tadalafil may be taken with or without food.

   Distribution:

         At therapeutic concentration, 94% of Tadalafil in plasma is bound to proteins.

   Metabolism:

            It is metabolized by the cytochrome P450 in the liver.

   Elimination:

         It is excreted in the feces(61%) and in the urine(36%).

   Half-life:

        17.5hours.

DOSAGE:

       Initial dose to be taken 30 minutes-12hours before sexual activity. Depending on the adequacy of the response or side effects.
